Our employee medical, dental and vision plan premiums have only increased once in the past four years
CVMC’s Overall Benefit Contribution = 30% of salary on average
Choice of HMO or PPO plan. Both plans include hospital/physician services and provide prescription drug coverage (including our in-house pharmacy benefit). CVMC pays 74% – 80% of premium cost.
100% coverage on preventative care, up to 4 cleanings per year allowed. Basic restorative coverage at 80%. Major restorative and orthodontics covered at 50%. Annual limit $1,500.00 per person. CVMC pays 50% - 65% of premium cost.
Plan includes exam and lenses each year, frames every other year. $10 co-pay on exam, $25 co-pay on materials. Additional discounts available.
403b Retirement Plan with 3% employer base contribution plus 3% employer match. Auto Enrollment feature for new employees. Pre-tax and Roth options available.
